Special Housing Requests due to Medical, Psychological,
and/or Physical Disability Needs
Housing & Residence Life works in conjunction with Disability Services, Health Services, and the Counseling Center to ensure consistency in evaluating special housing requests. Students interested in pursuing special housing based on medical, psychological or physical disability related conditions must submit documentation of the disability or condition from an appropriate health care provider. This person(s) must not have significant personal ties to the student or the student’s relatives beyond the professional relationship. The request form and required documentation must be received by the deadlines posted below.
Important Housing Deadlines for Special Housing Requests
Currently Enrolled Students:
Completed requests and all documentation
due by February 18, 2013
Incoming Students for Fall 2013:
Completed requests and all documentation
due by June 14, 2013

After on-campus professionals review the documentation provided, the student will receive notification of the final decision via his/her Furman University e-mail account. Students with approved needs will be offered high priority for placement in an available space that meets the need.

MEDICALLY-RELATED ONLY: CHECKLIST FOR STUDENTS WHO WISH TO REQUEST SPECIAL HOUSING DUE TO A DOCUMENTED MEDICAL NEED:
- Complete the Medical Need Special Housing Request form. This submission will be received by the Housing & Residence Life and Health Services Offices.
- The student’s treating physician should complete the Information from Health Care Provider (Medical Request) and mail to the address provided on the form.
- Complete the Authorization for Disclosure of Health Information and mail to the address provided on the form.
- All information should be received no later than the deadline posted above.
- Students with questions regarding the required information should contact Health Services at (864) 294-2180.
- If it is determined that a student is eligible for special housing, Housing & Residence Life will follow-up via e-mail regarding assignment options.
- Approval for special accommodations is for the coming academic year ONLY and updated documentation will be required each year.
PHYSICALLY-RELATED ONLY: CHECKLIST FOR STUDENTS WHO WISH TO REQUEST SPECIAL HOUSING DUE TO DOCUMENTED PHYSICAL DISABILITIES:
- Complete the Physical Disabilities Special Housing Request form. This submission will be received by the Housing & Residence Life and Disability Services Office.
- The student’s treating physician should complete the Information from Health Care Provider (Physical Disabilities) form and mail to the address provided on the form.
- Complete the Release of Medical/Confidential Information and mail to the address provided on the form.
- All information should be received no later than the deadline posted above.
- Students with questions regarding the required information should contact the Disability Services office at (864) 294-2320.
- If it is determined that a student is eligible for special
housing, Housing & Residence Life will follow-up via e-mail
regarding assignment options.
- Approval for special accommodations is for the coming academic year ONLY and updated documentation will be required each year.
PSYCHOLOGICALLY/MENTALLY-RELATED ONLY: CHECKLIST FOR STUDENTS WHO WISH TO REQUEST SPECIAL HOUSING DUE TO A DOCUMENTED PSYCHOLOGICAL/MENTAL HEALTH NEED:
- Complete the Psychological/Mental Health Need Special Housing Request form. This submission will be received by the Housing & Residence Life Office and the Counseling Center.
- The student’s treating professional should complete the Information from Health Care Provider (Psychological/Mental Health) form and mail to the address provided on the form.
- Complete the Authorization for Release of Information and mail to the address provided on the form.
- All information should be received no later than the deadline posted above.
- Students with questions regarding the required information should contact the Counseling Center at (864) 294-3031.
- If it is determined that a student is eligible for special housing, Housing & Residence Life will follow-up via e-mail regarding assignment options.
- Approval for special accommodations is for the coming academic year ONLY and updated documentation will be required each year.
Students will be notified of the status of their request within four weeks after the deadline date. Requests submitted after the deadline will be considered late and may result in a processing delay or lack of availability of the requested housing accommodations. New and/or urgent conditions will be considered upon receipt.
Please note that if a student with an approved need turns down the opportunity to live in a space that accommodates said need, the approval may be cancelled by Housing & Residence Life.
